Traits

  • Ghostly Mountainous Presence: The Mountain Worm Revenant can occupy the same space as other creatures and terrain, which is treated as difficult terrain.
  • True Poison: All poison damage dealt by this creature treats immunity as resistance and bypasses resistance completely.
  • Magic Resistance: The Mountain Worm Revenant has advantage on saving throws against spells and other magical effects.
  • feature: percieves time as if it was very slowe down, + 20 initiative and as one reaction per turn.
  • feature: 3 times per day the worm can as a reaction choose to revert the results of a offensive roll torwards itself, for exmaple conerting a critical hit into a cirirtical fail and making the attack miss. This is only for roll made by other creatures than the worm itself.
  • Undying Vengeance: If it dies, the corpse regains 50% of its HP and an additional 50% as temporary hit points. Its intelligence drops to one, and it attacks whatever killed it, then any other targets, dying after 1 hour regardless of its current HP.

Actions

  • Multiattack: The Mountain Worm Revenant makes three attacks: one with its bite and two with its tail.
  • Bite: Melee Weapon Attack: +18 to hit, reach 20 ft., one target. Hit: 36 (4d10 + 10) piercing damage plus 55 (10d10) poison damage. The target must make a DC 26 Constitution saving throw or be poisoned by Fleshwarp Toxin.
  • Tail: Melee Weapon Attack: +18 to hit, reach 30 ft., one target. Hit: 32 (4d8 + 10) bludgeoning damage. If the target is a creature, it must succeed on a DC 26 Strength saving throw or be knocked prone.
  • Constrict: Melee Weapon Attack: +18 to hit, reach 20 ft., one target. Hit: 42 (6d8 + 10) bludgeoning damage. The target is grappled (escape DC 26), restrained, and the Mountain Worm Revenant can't constrict another target until the grapple ends.

Description This terrifying aberration, formed from the corpse of a gigantic snake the size of a mountain, is a formidable challenge due to its sheer size and lethal poison that ignores immunities and resistances. This creature evolved from a magical corpse influenced by an intelligence from another reality, defying the natural laws of physics and magic.
